Midwifery Council Registration Numbers
All registered midwives have a Midwifery Council registration number. These were allocated by the Council when it assumed responsibility for the regulation of midwives, and replaces the old Nursing Council numbers. The format is 15- followed by five numbers. For example 15-54321. These numbers are considered confidential to the midwife concerned and are not given out by the Council.
Health Practitioner Index - Comon Person Numbers
Most currently practising midwives have now been added to the Health Practitioner index (HPI). This means that they have received a new number. This is the HPI Common Person Number, usually referred to as the CPN. Both the CPN and the Midwifery Council registration number will be printed on each midwife's practising certificate card. Newly registered midwives and midwives returning to practise after an absence will be added to the HPI at periodic intervals. In some cases they may receive their practising certificate card before the council is notified of their CPN number.
In the future it is expected that the CPN will be the only number that midwives (and other health practitioners) will use. However it will take some time for this to happen. Major users such as HealthPAC and ACC will need to adapt their systems before they can use this number.

Changing Your Name or Address
The Health Practitioner Competence Assurance Act 2003 requires midwives who change their name or their address to promptly notify the Council. It also stipulates that the Council holds more comprehensive address details than has been the custom in the past. This requirement continues to apply so long as you remain on the Register, even if you have stopped working as a midwife.
If you have changed your address you may use this change of address form to notify the Council. You will need to print it out and post it to us. Other forms of notification are also acceptable, including by telephone. In the latter case we may ask you some personal details to confirm your identity.
We cannot alter a name on the Register without formal evidence of a legal name change. If you have changed your name please send us a certified copy of your marriage certificate, deed poll, or statutory declaration with your request. A photocopy of any document must be certified as a true copy by a Justice of the Peace or Solicitor.
