Recently qualified midwives.
If you completed your Midwifery Programme this year and you have passed the National Midwifery Examination you are no longer a student. You cannot continue to attend women as a midwifery student ; if you do so you are a registered midwife practising without a practising certificate which is illegal. You must wait until you have your practising certificate. Applications are processed daily and you can check your status on the on-line Register.
Midwives intending to work as LMCs should also sign their contracts with the Ministry of Health to provide maternity services and access agreements with a DHB.
Practising Certificates for 2010-2011
Practising certificate renewal forms were posted to all midwives who hold a practising certificate for the 2009/2010 year, in late January to the postal address we have on the Register. If you have not received your form or you do not currently hold a practising certificate but wish to practice in the 2010-2011 year, please contact us and ask for an application form to be sent to you. Contact info@midwiferycouncil.org.nz or phone (04) 499 5040.

Changing Your Name or Address
The Health Practitioner Competence Assurance Act 2003 requires midwives who change their name or their address to promptly notify the Council. It also stipulates that the Council holds more comprehensive address details than has been the custom in the past. This requirement continues to apply so long as you remain on the Register, even if you have stopped working as a midwife.
If you have changed your address you may use this change of address form to notify the Council. You will need to print it out and post it to us. Other forms of notification are also acceptable, including by telephone. In the latter case we may ask you some personal details to confirm your identity.
We cannot alter a name on the Register without formal evidence of a legal name change. If you have changed your name please send us a certified copy of your marriage certificate, deed poll, or statutory declaration with your request. A photocopy of any document must be certified as a true copy by a Justice of the Peace or Solicitor.
